Population Health Initiatives
The health and well-being of our patient members is a top priority to HealthChoice. For that reason, our Population Health department focuses on initiatives to keep patients healthy. Our team of trained clinicians is available to offer lifestyle coaching and education, care management assistance, and complex care management for those who need it.
Credentialing
